Who is Served?
You are served if you are an adult with intellectual disabilities who is interested in day supports and you have an interest in participating in a group and community activities.
Individuals could be from age 18 and older because the program is dually licensed by the Department of Public Welfare’s Office of Developmental Programs and the Office of Long-Term Living.
Why is this Service Offered?
The Adult Day Program helps you learn skills so that you will be as independent as possible in your personal and social life.
What is the Service?
- You will be supported by professional staff and your team to develop and implement a plan that represents your interests and goals for the year.
- You will participate in a safe, comfortable, well supervised, and supportive environment.
- You will participate in activities facilitated by staff to further develop your communication and social skills, so that you can develop new friendships, as well as improve your interpersonal skills.
- You will be supported in increasing your everyday living skills, such as, social, leisure, academic, computer, fitness, horticulture and cooking skills.
- You will participate in community activities, such as bowling, parks, movies, restaurants, shopping, and volunteer opportunities. By doing this, you will learn more skills and build positive relationships with people in the community.
Services are provided in the community, in your home, or wherever assistance and support is required.
You are eligible to receive services if:
- You reside in Pennsylvania and are registered with Philadelphia Intellectual Disabilities Services (IDS). If you are not registered, PATH staff can help connect you with this governmental agency so that you can become registered.
- You receive approval and PATH receives authorization from Philadelphia Intellectual Disabilities Services (IDS) and Pennsylvania Office of Developmental Programs (ODP)
- This authorization allows PATH to provide this day service to you at no cost to you or your family.
