Benefits available Immediately upon first day of orientation:

If you are hired to work in some of PATH’s programs such as Residential, you may not have the option of taking off on holidays observed by PATH unless such holiday falls on your regularly scheduled day off. In such cases you will receive additional Holiday pay.

PENSION PLAN
To be eligible you must work at least 20 hours.
Eligible employees are enrolled in a 403(b) pension plan.
PATH contributes 3% of your annual salary at no cost to you. In addition, PATH will match 25% for every dollar that you contribute to the plan up to 10% of your base salary.
Vesting: 6 years = 100%, 5 years = 80% vested, 4 years = 60% vested, 3 years = 40% vested, 2 years = 20%. Any funds you contribute always remain yours.

BEREAVEMENT LEAVE
You are granted up to three (3) paid leave days for the death of an immediate family member. Immediate family members include parent or guardian, spouse, children, brother, sister, grandparent, mother-in-law, father-in-law or life companion.

LIABILITY INSURANCE
PATH carries comprehensive liability insurance for actions you take in the course of PATH business.

JURY DUTY
If you are called upon to serve, you will be paid the difference between your regular base pay and jury duty pay up to a maximum of 10 days.

Benefits available After Two (2) or More Months of employment:

HEALTH INSURANCE
PATH knows how critical it is for everyone to have excellent healthcare. You only need to work a minimum of 30 hours per week. We offer Keystone HMO, Personal Choice Flex, or Personal Choice HDHP. All plans include: Medical Services, Prescriptions, Dental & Vision. Some Employee Contribution is required.

PATH health care plans cover eligible employees and their families, as well as domestic partners with required documentation. Eligible the first of the month after 2 months of employment.

SICK LEAVE
If you are full-time, you earn 12 sick days per year.
Maximum accrual of sick time is 100 days; eligible after 90-days of employment.

If you are part-time, and work at least half of a full-time schedule, you accrue sick time in proportion to the number of your regularly scheduled hours.

PERSONAL TIME
If you are full-time, you earn:
4 days per year up to 10 years with PATH
5 days per year from 10 years – 14 years
7 days per year from 15 years – 20 years
9 days per year after 20 years with PATH

Eligible after 4-months of employment.

If you are part–time you will accrue personal time in proportion to the number of your regularly scheduled work hours, excluding sleep and overtime.

SUPPLEMENTAL LIFE INSURANCE
Supplemental life can be purchased upon date of hire or during open enrollement period; must work a minimum of 24-hours per week to be eligible.

Benefits available After six (6) or more Months of employment:

VACATION
If you are full-time, you will accrue as follows: 1st year: 10 days, 2nd to 4th year: 15 days, 5th year+: 20 days. If you are part-time and you work at least half of a full-time schedule, you accrue vacation time in proportion to the number of your regularly scheduled hours. Eligible after 6-months of employment.

LIFE INSURANCE
To be eligible you must work at least 30 hours.
You receive Life Insurance in the amount of your base annual salary. You receive Accidental Death and Dismemberment coverage. You may also purchase additional voluntary Life Insurance at group rates.

LONG TERM DISABILITY
Should you be unable to work because of a serious illness or serious accident you are paid 60% of your base salary upon approval of the Insurance Company. To be eligible, must work a minimum of 30-hours per week.

LEAVES OF ABSENCE
Family Medical Leave of Absence (FMLA) - Leave is granted because of the birth of a child, to adopt a child, to care for a family member who has a serious health condition, or due to your own serious health condition. You may receive up to 12 weeks of FMLA leave (eligibility begins after 12 months of employment and at least 1250 hours of work). Non-FMLA - Leave for illnesses not covered by the FMLA. Personal Leave of Absence - You may receive up to 3 months of leave due to circumstances of an exceptional nature.

Benefit Available After One (1) Year of Employment:

FLEXIBLE SAVINGS ACCOUNT FSA)
This voluntary program helps you pay for certain medical, dental, childcare, and other expenses on a pre-tax basis.
To be eligible, you need to work a minimum of 30 hours per week. Do not need to participate in PATH’s health insurance plan to participate. Eligible the first of the month after 2 months of employment.
