Intellectual Disabilities  Overview

PATH’s mission is: "Helping Individuals Achieve a More Independent and Fulfilling Life." PATH, for over thirty years, has provided a variety of services and supports to individuals with intellectual disabilities, and their families. In keeping with PATH’s mission to offer services that facilitate you becoming as independent as possible, you can receive services which range from those which are intensive and highly structured to those which offer minimal supports. Since your needs may change over time we offer flexibility in how we deliver services so that you can be supported as your needs change. All of PATH’s services are intended to maximize your personal growth toward independence within an atmosphere that stresses your dignity and self-worth. PATH employs a team approach in developing services and support which are tailored specifically for you. In addition to you, your team may consist of your family members, various PATH staff as well as support coordinators, outside consultants, specialized therapists, and others as appropriate. PATH believes that it is through this partnership that goals are achieved and dreams are realized.

While at PATH you will be treated consistent with the following principles:

  • You will be treated with dignity and respect by everyone at all times.
  • You will be treated in a confidential manner.
  • You will be supported and encouraged to participate in the planning of your own services and supports. You will have information about these services and supports, and you will decide about the supports and services you wish to receive.
  • You will be informed and consulted about any changes in your services, staff who work with you or program changes. This includes your right to refuse services.
  • You will receive the help necessary to understand all printed forms which you are asked to sign.
  • You can always ask about anything you do not understand or anything that makes you uncomfortable.
  • You will be supported in practicing you own cultural, ethnic or religious practices.
  • In residential services, you will be supported in managing your own financial affairs and making meaningful decisions about how you spend your money as well as your time.
  • Staff will use language and terminology that you and your family can understand.
  • You, your family and our staff will establish working relationships based upon mutual respect and understanding.
  • You may be asked for your permission to share information about yourself with others outside of PATH, in order for us to do a better job in working with you to reach your goals.
  • We ask that you communicate with us on a regular basis as to what you value and what you want so that we can work together to achieve your goals.
