Registered Nurse Residential Treatment Facility

Type: Full-Time (40 hours)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Registered Nurses will provide 24/7 on site coverage of a 12 bed adolescent PRTF and a small 4-6 bed young adult RTFA.  The Nurse is  responsible for oversight and provision of medication administration and medication monitoring; oversight and coordination of physical health of residents; provision of metabolic screenings; vitals; restraint oversight and following assessments and collaboration with RTF team including psychiatrist.

Specific Duties

  1. Be knowledgeable and comply with DPW’s 55 PA Code Chapter 3800 (Child Residential and Day Treatment Facilities Regulations) and OMHSAS’s memo date November 1995:  Requirements for Residential Treatment Facilities for Adults.
  2. Be knowledgeable and comply with CARF, CBH, PRTF and DHS requirements/regulations.
  3. Provide and ensure all aspects of Medication Administration and Medication Monitoring, to include trainings, coordination with pharmacy and collaboration with RTF psychiatrist are completed in a timely, comprehensive manner.
  4. Complete Metabolic Screenings and other assessments/screenings (e.g. AIMS) on each resident.
  5. Provide oversight of physical health needs for each resident, including ensuring any dietary needs are met.
  6. Complete health histories, as assigned.
  7. Provide vitals and medical intervention, as indicated.
  8. When indicated, coordinate PCP, Urgent Care or Emergency Room visits.
  9. Obtain orders for restraints, oversee restraints and provide follow up assessments after a restraint.
  10. Collaborate with the RTF psychiatrist on any medication and/or evaluation needs
  11. Provide therapeutic interventions for the residents including crisis de-escalation techniques; conflict resolution skills and improvement of communication skills.
  12. Order and maintain medical supplies.
  13. Be familiar with and administer Narcan, oral swabs, breathalyzer and other diagnostic tools as indicated.
  14. Provide program coverage, as assigned.
  15. Complete and assure that all required documentation in a comprehensive, timely and legible manner and in accordance with PATH policies and procedures.
  16. Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.

Qualifications

Successful completion of RN program and an active PA RN license in good standing.  At least 1 year experience in behavior health care.  Must have valid driver’s license and personal vehicle. Excellent interpersonal skills.
