Registered Nurse - Behavioral Health/Children/Youth Urgent Care Center

Type: Full-Time (40 hours)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. To work as part of the interdisciplinary team to provide crisis intervention, support, safety plans and linkages to children, youth, and families in Northeast Philadelphia.  Perform RN duties for UCC; maintain medical supplies; and other assigned duties to assist physicians in meeting child / youth and family needs.  The UCC program operates 5 days/week Monday through Friday between the hours of 11:00am-7:30pm.

Specific Duties

  1. To provide crisis management that is coordinated and integrated with the Interdisciplinary treatment team of the UCC.
  2. To provide telephone crisis and triage coverage during hours of operation in order to arrange for assessment, make referrals to appropriate services, or provide education and support of ongoing care.
  3. Engage the youth and family upon visit to UCC. 
  4. Take vital signs and complete health history including medications, medical conditions and allergies.
  5. When indicated, coordinate visit to emergency room for medical clearance.
  6. Be familiar with and administer Narcan and, UDS, utilizing breathalyzer, Oral swabs and other diagnostic tools, as indicated.
  7. Coordinate efforts directed toward meeting the needs of children, youth and families who present to UCC.
  8. Collaborate with the psychiatrist regarding evaluation needs.
  9. Call medication into pharmacy as directed by physicians.
  10. Provide liaison services as follow-up to UCC visit as requested by psychiatrist.
  11. To complete or assist others in completing the involuntary commitment petitions.
  12. To safely deescalate crisis, only using approved therapeutic holds when a youth is engaging in behavior that presents a danger to self or others.
  13. Collaborate with the crisis manager on resources and linkages needed and assure the linkages are in place.
  14. Coordinate transportation to UCC at 9pm, if indicated.
  15. To continue your shift in the UCC until your replacement arrives or until your supervisor gives you approval to leave.
  16. Participate in training and on going discussion regarding the norms and values of the variety of ethnic and cultural groups in Northeast Philadelphia.
  17. Obtain language/ translation services as needed to work effectively with youth and families.
  18. Review and assure compliance with all documentation requirements.
  19. Order and maintain medical supplies and medications, ensuring that expired medications are disposed per Path policy.
  20. Meet with pharmaceutical representative as assigned.          
  21. Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.
  22. Obtain supplies from pharmacy as needed.
  23. Keep strict account of number of medications ordered and administered for control substances.
  24. Review with medical director prior to order medications from pharmacy.
  25. Keep office organized and clean.


Successful completion of RN program and an active PA RN license in good standing.  At least 1-year experience in the provision of nursing care.  Experience working in Behavioral Health Care.
