Psychosocial Rehabilitation Specialist (Renaissance)

Type: Full-Time (40 hours per week)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member.   To utilize a variety of approaches and interventions including the Boston University Approach to Psychiatric Rehabilitation and Motivational Interviewing to promote recovery within a site based program as well as provide successful community integration experiences.  Provide individualized support and skill teaching in the community to promote new role development.  Assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual.

Specific Duties

  1. Maintain individual caseload under supervision of Program Manager
  2. Actively engage individuals in services including outreach visits at the individual’s home, crisis center or outpatient facility
  3. Engage and support individuals in all phases of the psychiatric rehabilitation process
  4. Assist individuals in developing and practicing the skills defined on their psychiatric rehabilitation plans which may include accompanying individuals to appointments, leisure activities or other activities of interest such as religious, educational or vocational activities
  5. Link individuals to community resources and peer supports to promote community integration
  6. Collaborate with involved personnel in the community such as GED instructors, college professors, faith-based representatives, volunteer and employment supervisors as assigned by Program Manager
  7. Responsible for developing psychiatric rehabilitation and treatment plans, progress notes, annual psychosocial evaluations
  8. Consult and collaborate with all team members including psychiatrist on a regular basis
  9. Assist in the development and implementation of psycho-education, domain related and support groups deemed beneficial by the Stakeholder Committee
  10. Meet community integration projections
  11. With the individual’s consent, engage family members, or other members of the individual’s identified support system
  12. Assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual
  13. Provide 24 hour/7 days a week crisis coverage on a rotating basis
  14. Develop effective crises sheets for assigned individuals and revise as changes occur
  15. Take appropriate action during a medical incident including calling 911 in community, at the site or when an individual is calling from an alternate location
  16. Draft accurate and timely behavioral health review minutes as assigned
  17. Ensure timely updates of all Medical Assistance Transportation Program forms
  18. Drive PATH vehicle for program reasons and  follow protocols related to PATH vehicles
  19. Develop a graduation plan with assigned individuals as appropriate including linkages to clinical and support services
  20. Complete discharge documentation in a timely fashion
  21. Obtain Certified Psychiatric Rehabilitation Practitioner (CPRP) within 18 months of employment
  22. Be aware of financial liabilities, coordinate any financial changes with the billing department and assist in securing their party resources when indicated
  23. Participate in quality improvement and data gathering as assigned
  24. Complete all required documentation in accordance with PATH policies and procedures


Bachelor’s Degree in Human Services or Rehabilitative Services from an accredited college or university.  Experience working with individuals who have a mental illness or a co-occurring disorder.  Driver’s license & use of personal vehicle required. Good organizational skills.

$35,339 annually
