Program Manager (CMIS)

Type: Full-time (40 hours)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member.  Responsible for supervision of CMIS staff and services provided to families served in CMIS program in Northeast Philadelphia.  CMIS provides short term, up to 6 weeks, crisis management and therapeutic services for children and their families.  Such services include assessment, crisis intervention, parent education, brief resolution focused treatment and linkages to behavioral health and community services 365 days/year with staff providing direct response 7 days/week from 8:00am to 9:00pm

Specific Duties

  1. Provide weekly individual clinical supervision to assigned intervention specialists and case managers to ensure appropriate use of identified evidence based practices. 
  2. Join the intervention specialist during one of the initial 2 sessions with each family to assist with case formulation, treatment planning, implementation of interventions, conflict resolution and crisis management.
  3. Provide and assure staff receive all required trainings
  4. Promote family sustainability and assure services are family driven
  5. Assist and ensure collaboration between intervention specialist, case manager and psychiatrist
  6. Review each youth and family with staff and assist in determining if additional behavior health and/or community referrals are indicated.
  7. Assure appropriate linkages and collaboration with other service systems occurs in a timely, collaborative manner. 
  8. Train and support staff in the development and use of identified structured evidence based tools.
  9. Ensure staff receives training in the norms, values of the variety of ethnic and cultural groups in Northeast Philadelphia and assist with any language linkages or resources needed to work effectively with youth and families.
  10. Provide on call coverage on a rotating basis to provide support and guidance to Mobile Crisis BHW’s during the hours of 10:00pm and 8:00am.
  11. Provide direct crisis support and intervention during times of surges in calls
  12. Review and assure compliance with all documentation requirements.
  13. Develop and monitor staff schedules to assure adequate coverage at all times. 
  14. Assure any follow up appointments are scheduled at a time and place convenient for families, which includes the provision of evening and weekend appointments in the home and community.
  15. Assist in the development and oversight of the rotating 24/7 on-call schedule
  16. Complete all required documentation including assessments, annual reviews, progress notes and discharge summaries in a comprehensive, timely and legible manner and in accordance with PATH policies and procedures.
  17. Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.
  18. Additional responsibilities as assigned

Qualifications

A minimum of a Master’s degree in the human services field from an accredited college or university.  A minimum of 2 years verified, paid, post-master’s therapy experience with children/youth and training and experience in family therapy.  Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist or Licensed Psychologist required. Valid driver’s license and vehicle required.  Fluency in at least 2 languages spoken in Northeast Philadelphia
