Program Manager (CMCT)

Type: Full-time (40 hours)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Responsible for CMCT team oversight and the provision of clinical services to youth and families served in CMCT in Northeast Philadelphia (Region 1).  Such services include assessment, crisis intervention, and linkages to behavioral health and community services 365 days/year with staff providing direct response 7 days/week from 8:00am to 10:00pm and a rotating on-call system from 10:00pm to 8:00am.

Specific Duties

  1. Provide weekly individual clinical supervision to assigned Behavior Health Workers (BHW) and Family Advocates to ensure appropriate use of identified evidence based practices. 
  2. Provide and assure staff receive all required trainings
  3. Join BHW’s and Family Advocates in responding to crisis calls to provide support, supervision, training and modeling.
  4. Review each crisis contact with staff and assist in determining if additional behavior health and/or community referrals are indicated.
  5. Assure appropriate linkages to behavior health and/ or the Mobile Crisis Intervention Services occurs in a timely, collaborative manner. 
  6. Train and support staff in the development of safety/ crisis plans and use of identified structured evidence based tools.
  7. Provide on call coverage on a rotating basis to provide support and guidance to BHW’s during the hours of 10:00pm and 8:00am.
  8. Provide direct crisis support and intervention during times of surges in calls
  9. Assure all crisis staff respond to crisis calls in the designated time period
  10. Review and assure compliance with all documentation requirements.
  11. Develop and monitor staff schedules to assure adequate coverage at all times. 
  12. Assure any follow up appointments are scheduled at a time and place convenient for families, which includes the provision of evening and weekend appointments in the home and community.
  13. Assist in the development and oversight of the rotating 24/7 on-call schedule
  14. Complete all required documentation including assessments, annual reviews, progress notes and discharge summaries in a comprehensive, timely and legible manner and in accordance with PATH policies and procedures.
  15. Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.
  16. Additional responsibilities as assigned

Qualifications

A minimum of a Master’s degree in the human services field from an accredited college or university.  A minimum of 2 years verified, paid, post-master’s therapy experience with children/youth and training and experience in family therapy.  Licensed LMFT; LCSW, LPC or Licensed Psychologist required.  Valid driver’s license and vehicle required.
