Health Information Services (HIS) Clerk

Type: Full-Time (37.5 hours per week)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member of the Health Information Services (HIS) Department.

Specific Duties

  1. Enter data from Service Activity Logs into the Avatar system in the order identified as determined by the Health Information Services Manager on a timely basis.
  2. Identify omissions on Service Activity Logs before they are entered into the Avatar system.
  3. Contact Program Staff to obtain data omitted from Service Activity Logs.
  4. Print and distribute all Service Activity Logs to all Programs.
  5. Verify receipt of Service Activity Logs on a daily basis.
  6. Review the Service Activity Log Control Report on a weekly basis.
  7. Contact the Program Supervisor/Manager to obtain any outstanding/late Service Activity Logs identified by the weekly review of the Service Activity Log Control Report.
  8. Notify the Health Information Services Manager or Chief Information Officer if unable to obtain outstanding Service Activity Logs after contacting Program Supervisor/Manager.
  9. Maintain the paper files of processed Service Activity Logs in the established manner.
  10.  Sort and file all processed Service Activity Logs by Staff ID and Month/Year.
  11. Maintain the electronic files of scanned Service Activity Logs in the manner identified by the Health Information   Services Manager.
  12. Scan all processed Service Activity Logs into the system.
  13. Transfer and organize all scanned Service Activity Logs into folders by Month/Year and Staff ID.
  14. Retrieve and print scanned Service Activity Logs as requested by other Staff.
  15. Retrieve and copy paper Service Activity Logs that have not been scanned as requested by other Staff.
  16. Assist in identifying issues as related to Health Information Services activities and suggest resolutions.
  17. Provide back-up for the Health Information Services (HIS) Assistants as determined by the Health Information Services Manager.

Qualifications

High School Diploma or GED required.  Training or experience in data entry.  Proficiency in entering data through use of number keypad and typing.   Ability to operate office equipment including copiers, faxes, printers, etc.

$25,584.00 per year ($13.12 per hour)
