Community Inclusion Specialist - Renaissance

Type: Full-Time (40 hours per week)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by supporting the Renaissance team to provide quality services in the community.  Responsible for the overall development and leadership of PATH’s Renaissance Services Community Participation Activities in accordance with PATH’s mission, values, and principles.

Specific Duties

  1. Research community inclusion activities and volunteer opportunities to help create an engaging program based upon the needs, preferences, and abilities of Renaissance participants.
  2. Create a community activity and integration program based upon the needs, preferences, and abilities of Renaissance participants.
  3. Find and facilitate creative community inclusion events that use and incorporate a variety of modalities, including but not limited to arts and crafts; drama, music, and dance, sports and games and aquatics.
  4. Coordinate and provide an ongoing, highly-organized program of community activities which stimulates and promotes the physical, spiritual, social, emotional, and intellectual well-being of Renaissance participants.
  5. Continuously seek out potential new activities and organizations/foundations to create relationships and opportunities for Renaissance participants including Settlement Music School, NORCOM, the Philadelphia Mural Arts Program and the like.
  6. Facilitate membership in Artreach and ensure that it is utilized well including coordinating independent activities with participants.
  7. Develop cohorts of individuals to participate in community inclusion activities that have similar interests, desires and capabilities.
  8. Communicate and follow up with Program Managers and Psychosocial Rehabilitation Specialists regarding the facilitation of various events ensuring that all have knowledge of the monthly calendar of events and added specialized activities.
  9. Provide activities and opportunities for social engagement according to the seasonal and religious preference of the individuals.
  10. Be physically present, as needed to ensure the success of various activities.
  11. Coordinate and Consult with Renaissance management around transportation, accommodations, and event related issues.
  12. Coordinate Family Fun Night events.
  13. Coordinate on site activities or clubs for participants.
  14. Work with the team to assess each individual to develop individualized community integration/inclusion goals and activities.
  15. Attend Renaissance Case Consultations and provide feedback from your discipline’s perspective which will support an individual who is in an emerging behavioral health situation/crisis or has been hospitalized and a plan is being developed to avoid re-hospitalization.
  16. Purchase and maintain all supplies necessary to ensure availability for a vibrant on-site activities program.
  17. Assist with preparation for annual State Inspection for Licensing and assure team’s compliance to regulatory requirements. 
  18. Meet regularly for supervision with Renaissance Program Director.

Qualifications

Bachelor’s Degree in Recreational Management, Recreation Administration, Recreational Therapy preferred or other related human services field; At least two years of experience working directly with people with behavioral health issues, physical disabilities or intellectual disabilities or any appropriate combination; Proficient computer skills; Excellent writing skills
