Children's Blended Case Manager

Type: Full-Time (40 hours per week)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member.  Work primarily with children and adolescents with serious mental illness or emotional disorders and their families, with the goal to maintain the youth in the least restrictive setting with regards to home, school and community programs.  Work closely with youth and their families in providing the support, training and assistance required for a stable and healthy family and community life.

Specific Duties

  1. Provide a comprehensive strengths based assessment for each assigned child/youth
  2. Develop with each child/youth and family, a unified service plan addressing the needs for treatment, skill development, social and emotional development and social service needs
  3. Ensure that the principles developed by Pennsylvania Child and Adolescent Service System Program (CASSP) are followed in providing services for youth and their families
  4. Coordinate psychiatric services as indicated for assigned children/youth
  5. Attend meetings with residential, school and hospital staff to coordinate resources and services for assigned children/youth
  6. Assist the child/youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation, education.
  7. Coordinate continuity of care for each assigned child/youth
  8. Monitor the continued need for BCM for each assigned child/youth and assist him/her in moving to different levels of care as indicated
  9. Provide home, community and school visits to each assigned child/youth and family
  10. Monitor service delivery by maintaining contact with assigned children/youth, families and other service providers involved with the youth/family
  11. Complete a face to face contact with each assigned child/youth in accordance with DBH guidelines
  12. Actively participate in discharge planning for assigned child/youth, including if a child/youth is hospitalized
  13. Complete all paperwork in a timely, comprehensive, legible manner and in accordance with PATH policies and procedures
  14. Advocate for assigned children/youth and families as needed
  15. Attend all meetings related to assigned children/youth, as well as other assigned meetings
  16. Respond  to all emergencies and crises of assigned children/youth and mobilize available resources to assist the child/youth and family in crisis
  17. Provide 24 hour crisis coverage for eligible children/youth as assigned and respond to each call within 15 minutes
  18. Be aware of family’s financial liabilities and coordinate any financial changes with PATH’s Billing Department, and assist families in securing third-party resources when indicated
  19. Meet agency productivity standards of 75% for your position


Bachelor’s Degree in Human Services field from an accredited college/university.  One to two years experience in the behavioral health field.  Experience working with children and adolescents preferred.  Use of own personal vehicle and valid driver’s license.  Good interpersonal and organizational skills.
