Case Manager-Family Based Mental Health Program

Type: Full Time (40 hours per week)

Job Summary

To work as a team member to provide intensive family based therapeutic services, delivered primarily in the family’s home to include:  assessments, co-therapy service coordination and linkages, crisis intervention and prevention, educational/daily living skills building for assigned families in an effort to prevent out of home placement.

Specific Duties

  1. Provide initial and on going comprehensive assessments of identified families.
  2. Complete any required intake/registration documentation.
  3. Work collaboratively with assigned Therapist to provide comprehensive services and linkages for identified families.
  4. Work with therapist in providing family therapy to identified families.
  5. Complete all required documentation in a timely, legible manner.
  6. Provide 24/7-beeper coverage on a rotating basis.
  7. Deliver services in a manner convenient to the family and the family’s needs, this includes evening and weekend service provision.
  8. Consult with Child Psychiatrist on a regular basis.
  9. Collaborate with all systems involved with the family.
  10. Provide linkages to all needed services for the family.
  11. Provide on going role modeling to family.
  12. Provide behavior intervention training/support to families.
  13. Develop and implement socialization and activities for families.
  14. Be aware of and follow Path policies and procedures including those regarding confidentiality of treatment.
  15. Participate in team meetings and case conferences.
  16. Participate in supervision, trainings and other assigned committees, or task forces.
  17. Be aware of and follow all funding source documentation, reporting, and service delivery requirements.
  18. Contribute to any quality improvement/outcome measurement tracking, development, data gathering as assigned.
  19. Attend required ecosystemic family therapy trainings.
  20. Other related duties as assigned.

Qualifications

Bachelors Degree in Social Work, Counseling, Marriage and Family or Psychology - 1 year experience in a CASSP System Program.

Salary

$32,240.00 (15.50 / hour)
