Community Livings Arrangements - Residential Site Supervisor

Type: Full-Time (40 hours per week)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by managing an effective team to provide quality services.  Responsible for the overall leadership and supervision of PATH’s Community Living Arrangements Sites in accordance with PATH’s mission, values, and principles.

Specific Duties

  1. Provide supervision of assigned residential sites
  2. Maintain and enforce compliance with DPW/ODP regulations.
  3. Conduct Site Orientation of new staff to assure that staff are knowledgeable about residents needs/services
  4. Regularly observe and provide feedback/training to Residential Skills Trainers to ensure that their interactions with each resident are consistent with the individual’s goals, uses techniques and approaches that positively supports individual’s ISP, as well as ODP’s Everyday Lives Philosophy
  5. Management of all site related issues: Support of residents, ensure cleanliness and orderliness of sites, regular employee feedback, evaluations and when necessary, trainings.
  6. Monitor assigned residential sites to ensure health and safety of residents and inform PATH’s Facilities Department of any facility problems in a timely manner.
  7. Assure that residents engage in community integrated activities in keeping with their expressed interests and their goals whether ISP based or personal preference based. 
  8. Provide direct services as needed, especially during staff vacancies in consultation with scheduler
  9. Attend and facilitate monthly meetings: site meetings and Support Coordinator Monitoring, following up on monthly monitoring concern sheets with supervisor and Program Specialist Manager
  10. Provide 24hour, 7 day a week On-Call supervisory coverage on a scheduled basis which includes responding to phone calls in accordance with PATH On-Call Policy and Procedures and providing face to face intervention as indicated to resolve a crisis
  11. Appropriately schedule and ensure proper usage of agency/program vehicles
  12. Conduct site visits and review relevant paperwork, e.g. communication logs, petty cash records, medication logs, monthly reports, site checklists, and health promotion forms
  13. Participate in Human Resource related issues including disciplines, write ups, investigations, and coordination of staff drug testing
  14. Track petty cash/Pay resident bills/Process check requests/facilitate spend downs as needed
  15. Complete Residential Skills Trainer interviews as scheduled
  16. Inform supervisor and other CLA and PATH supervisory staff of all relevant issues consistent with PATH and City/State Policies to assure the health and safety of those we serve.
  17. Reconcile electronic time keeping exceptions daily, approve timesheets, work in conjunction with the CLA Scheduler to assure that all shifts are appropriately provided and the health and safety of those in serve are maintained.
  18. Assure the completion and processing of Incident Reports occurs within PATH established time lines.
  19. Assure Fire Drills are completed per regulations.
  20. Coordinate with Healthcare regarding residents’ appointments, attend medical/psych appointments as needed, coordinate medication pickups and collect empty blister packs
  21. Maintain Program/Financial charts at sites and update site SOP book consistent with PATH Policies and Procedures.
  22. Reconcile financial chart audits in coordination with CLA Accountant.
  23. Assist with payeeship application for Social Security Benefits
  24. Obtain consumer signature for HUD documents
  25. Liaison with medical and rehabilitation facilities and assure coverage in coordination with CLA Scheduler 
  26. Work closely with Program Supervisors to coordinate and provide quality clinical care
  27. Assist in residents’ celebrations and client related issues, such as but not limited to Birthday/ retirement events, voter registration and funeral planning.
  28. Meet with Behavior Specialist as needed to improve clinical outcomes for residents
  29. Ensure all consumer goals and health promotion activities are being implemented in keeping with clinical documentation.
  30. Complete 6400 Pre-licensing Surveys in conjunction with Program Specialist
  31. Perform other duties as assigned

Qualifications

High School Diploma/GED plus five years of experience, or Associate’s Degree, or sixty (60) credit hours and four (4) years experience working with persons with intellectual disabilities, or Bachelor’s Degree with (2) years experience working with individuals with intellectual disabilities, or Master’s Degree and one (1) years experience working directly with persons with intellectual disabilities.  Driver’s License and use of own personal vehicle required. Must have excellent writing, analytical, presentation, clinical and interpersonal skills.  Excellent clinical, communication and interpersonal skills, excellent writing skills, thorough knowledge of Intellectual Disabilities Field and Best Practices.  Valid driver’s license and vehicle required.

$36,774.00 - $41,454.00 annually
Salary commensurate with degree & experience
