Blended Case Manager - Adult

Type: Full-Time (40 hours per week)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member.   Case Managers are responsible for assisting individuals to make full use of Behavioral Health Services and natural community supports which will help individuals achieve a more independent and fulfilling life.  Services provided will include developing a partnership with the individual to assess needs, obtain services and resolve crises.  Through this partnership the individual and case manager work together to plan, coordinate, monitor and advocate for the services and supports needed to achieve the individual’s goals. 

Specific Duties

  1. Develop partnerships with assigned individuals to assess needs, obtain services and resolve crises
  2. Assist individuals with the development of a Personal Goal Plan
  3. Assist individuals with identifying a goal and defining the steps needed to achieve the goal
  4. Complete all assessment and planning documentation including Adult Psychosocial Assessment; Strengths Assessment; Treatment Plans and other required documentation
  5. Ensure all required documentation is completed in a timely, comprehensive and legible manner and is in compliance with PATH policies and procedures
  6. Provide 24 hour/7 days a week crisis coverage as assigned
  7. Provide weekend staffing as assigned
  8. Assist in linking individuals with their chosen services and resources in the community such as needed psychiatric and medical treatment, residential services, financial entitlements, social/recreational activities and general social services
  9. Monitor those supports and services for appropriateness and effectiveness
  10. Provide informal support and network building to help individuals increase their support system outside of the Behavioral Health System
  11. Advocate on behalf of individuals to assist them in accessing appropriate services
  12. When working with specialized populations such as Young Adults or individuals who speak Russian as a primary language, stay abreast of age appropriate and/or culturally relevant services
  13. Meet or exceed 75% productivity requirements for your position
  14. Attend internal meetings such as team meetings, clinical case conference and program meetings as well as attend meetings outside of PATH as assigned
  15. Be aware of financial liabilities; coordinate any financial changes with the billing department and assist individuals in securing third party resources when indicated
  16. Attend internal and external trainings on relevant topics as assigned


Bachelor’s Degree in Human Services with preference for 1-2 years experience in the Behavioral Health field. Valid driver’s license; ability to meet agency insurance requirements; and use of personal auto.
