Behavioral Health Worker - Crisis

Type: Full Time (40 hours)

Job Summary

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member.  To work as a team member to provide crisis intervention, support, safety plans and linkages to youth and families in Northeast Philadelphia, Region 1.  The program operates 365 days/year from 8:00am – 10:00pm Mondays through Fridays and 11:00am – 7:00pm on Saturdays and Sundays.  Rotating on-call coverage from 10:00pm – 8:00am.

Specific Duties

  1. Provide crisis assessment, intervention, education and support in person, with assigned team  member, within a designated time frame to referrals received by DBH/CBH.
  2. Utilize approved evidence based practices and tools when providing crisis assessment and intervention, including SAMHSA crisis intervention guidelines
  3. Assure all interventions are family focused and family driven and resolution focused.
  4. Assist the family in the understanding of the events that led to the crisis from everyone’s perspective.
  5. Assist the child/youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation and education.
  6. To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed and the plan has been developed
  7. Assure appointments are scheduled at a time and place convenient for the family.
  8. To complete or assist others in completing involuntary commitment petitions.
  9. To arrange and follow through with voluntary or involuntary hospitalization admissions.
  10. Develop with the youth and family a safety/ crisis plan obtaining each individual’s perspective, strengths and ideas
  11. Link the youth and family to the CRC or Urgent Care Center and remain with the youth and family until the connection and collaboration is completed.
  12. Provide rotating on call crisis coverage during the hours of 10:00pm – 8:00am to include phone crisis assessment, intervention and linkages.  If in person contact is indicated, review with on call supervisor before any implementation.
  13. Be knowledgeable of local community resources and services and provide care coordination.
  14. Provide up to 72 hours of intervention by scheduling follow up appointments at a time convenient for the family, which includes evening and weekend appointments in the home and community.
  15. Provide targeted one to one support of identified youth as directed.
  16. Participate in trainings and on-going discussions regarding the norms and values within the variety of ethnic and cultural groups in Northeast Philadelphia.
  17. Obtain language/translation services as needed to work effectively with youth and families.
  18. Participate in clinical team meetings and other meetings as assigned.
  19. Complete all required documentation in a timely, comprehensive and legible manner and in accordance with PATH policies and procedures.
  20. Attend all meetings related to assigned children/youth, as well as other assigned meetings.
  21. Contribute to any quality improvement/outcome measurement tracking and data gathering as assigned.

Qualifications

Bachelor’s degree in social work, counseling, marriage and family or psychology from an accredited college or university.  2 years experience in the CASSP system.  Good interpersonal skills.  Must have valid driver’s license and personal vehicle.
